About Case Management Redesign

RMHS is working to answer all questions as quickly as possible.

Due to recent changes to Colorado’s Home and Community Based (HCBS) case management system, we are experiencing a higher volume of calls and longer wait times. We apologize for any inconvenience this may have caused. The State of Colorado’s Department of Health Care Policy and Financing (HCPF) has acknowledged the challenges of the CMRD process in a letter to members and reaffirms their commitment to addressing these issues.

At RMHS, our priority focus is providing you with the support you need in a timely manner. If you have questions about your home and community-based case management services and have had trouble connecting with someone in our call center, you can submit your inquiry via our online form and someone will connect with you.

What is Case Management Redesign

Case Management Redesign is the state’s plan to make it easier for people in Colorado to get the help they need and make it simpler to find the right services.

On November 1, 2023, RMHS became the agency in charge of providing case management for people in Denver and Adams Counties, and Developmental Pathways is in charge of serving people in Douglas, Arapahoe, and Elbert Counties. Map of Case Management Agencies

What does this change mean for me? Which agency is my Case Management Agency?

The first step is to determine which county you live in.

If you live in Douglas, Arapahoe, or Elbert Counties, your case management services automatically switched to Developmental Pathways on November 1st, and they assigned a new case manager to help you.

If you live in Denver or Adams Counties, RMHS is your case management agency.

2. Call the RMHS main line (303-636-5600) and use the phone directory to find and contact your case manager.
