RMHS provides case management for Long-Term Services and Supports in Denver and Adams Counties for children and adults with various types of disabilities, including intellectual and developmental disabilities (I/DD), physical and functional impairments related to brain injuries, spinal cord injuries, mental health challenges, aging adults, and more.

What to Expect From Case Management at RMHS
Our team of case managers work with people to assess program eligibility and provide service options that best meet their long-term support needs. After enrollment, RMHS case managers provide ongoing case management services through annual in-person assessments, service planning, and regular touchpoints to ensure that each person receives the Long-Term Services and Supports that are just right for them.
We take a person-centered approach, helping people understand their options and facilitate connections to services and providers, such as:
- Adult day services
- Home delivered meals
- Modifications to homes for accessibility
- Medication reminders
- General support such as dishwashing, laundry, shopping, bathing, dressing, and much more
- Options for alternative community living
- Transportation to places other than medical appointments
- Respite
- And more
Our team of case managers helps monitor these services so that individuals receive the right Long-Term Services and Supports when they are needed.
Long-Term Services and Supports Offered by Medicaid
- CES Wavier: Services for children with a developmental disability and significant medical or behavioral needs who require constant line of site
- CHCBS Waiver: In-home support for children with significant medical needs and at risk of institutional care
- CHRP Waiver: Residential support for children with I/DD and very high needs, putting them at risk of out-of-home placement
- CLLI Waiver: Counseling, therapy, and respite services for children with life-limiting illnesses
- FSSP: Individualized support for families caring for a child in the home with I/DD
- BI Waiver: Support for people with a brain injury who need extra support to live in their community
- CIH Waiver: Supports such as acupuncture, chiropractic services, and massage therapy for people living with a spinal cord injury, multiple sclerosis, brain injury, spina bifida, muscular dystrophy, or cerebral palsy
- CMHS Waiver: Services such as day services, home modifications, and delivered meals for people with a mental illness that require long-term supports to remain in a community setting
- DD Waiver: 24-hour support for adults with developmental disabilities (such as Down syndrome, autism and cerebral palsy) who need extensive support to live in their community
- EBD Waiver: Long-term support for people ages 65 and older who have a functional impairment, are blind, or for people ages 18-64 who are physically disabled or have a diagnosis of HIV or AIDS
- HCA: Cash assistance to fund a home care provider to support people aged five and older with a disability
- OBRA: Integrated community-based support for individuals with I/DD living in a nursing facility
- PACE: Enabling older adults to live in their community of choice as long as it is medically and socially feasible through services such as adult day services, dental services, emergency services, home care services, hospital care and much more
- SLS Waiver: Support with daily living activities for adults with I/DD who live at home. Not a 24-hour program
- HBU: Hospital-level care to people in nursing facilities who are on a ventilator, have complex wounds, or have medically complex needs
- LTHH: Short-term or long-term home health services for people experiencing acute or long-term health conditions
- NF: Assesments for entry into skilled health care institutions that provide medical care and rehabilitation services.
